| About the Permanent Supportive Housing Program (PSHP)
The Permanent Supportive Housing Program (PSHP) is an initiative developed to provide permanent housing and supportive services to chronically homeless individuals and families. The program assists individuals and families through housing stabilization, maximizing levels of self-sufficiency and improving overall quality of life.
Program Phases
The program is broken into three phases. The first phase identifies individuals and families with a history of homelessness who are living on the streets, in shelters or other institutions. The second phase places individuals and families into long-term housing. Finally, the third phase provides case management to ensure participants are connected to needed support services.
Objectives
The goal of the PSHP is to create the enabling conditions for program participants to achieve the following objectives:

Obtain long-term housing

Maintain housing by complying with lease provisions and local laws

Achieve the highest level of self-sufficiency possible and improve the overall quality of their lives
Case Management
The case management services will be provided in a way that creates the enabling conditions for individuals and families to achieve the highest degree of stabilization and self-sufficiency possible. Listed below are some of the responsibilities of case managers.

Assist program participants with housing placement.

Develop and revise service plans for (and in collaboration with) program participants.

Refer program participants to needed supportive services to achieve service plan goals.

Ensure that program participants are receiving and engaged in needed supportive services.

Coordinate, monitor and evaluate supportive services provided to program participants.

Monitor and track program participant’s progress towards service plan goals.

Monitor program participant’s compliance with their housing lease and PSHP rules.

Monitor program participant’s general health and safety.

Serve as advocate for program participants.

Serve as mediator/liaison between program participants, their landlords and service providers.
